Game Reports

When does the center referee have to send in a game report and supplemental report?

Answer: For CJSA youth soccer games, any time you have either:

                a) a send-off (i.e. red card)

                b) a serious injury to report

                c) some strange incident (like a tree falling onto the field, or any
strange or unusual interruptions of the game like a
thunderstorm, an intruder, a fight that caused a lengthly
disruption or even termination of the game, etc.)

then you must fill out a game report and a supplemental report.

Also, all CT State Cup games and Premier league matches
require a game report and supplemental report.

* IN THE CASE OF A SEND-OFF PLEASE REMEMBER TO WITH-HOLD THE PLAYER(S) PASSES AFTER THE GAME.  The game report, supplemental report and player passes of ejected players should be MAILED IN TO the CJSA head office in a stamped envelope and sent to:

Connecticut State Referee Program
11 Executive Drive
Farmington, CT
06032

You can also send the game and supplemental reports via e-mail to info@ctreferee.net, in which case it's a good idea to copy yourself on the e-mail.  But the passes still must be mailed in by snail mail.

* Also, since the game report will ask you for the NAMES and GRADES of the referees that assisted you with that game, it's a good practice for the center referee to always RECORD THE FIRST AND LAST NAMES OF THE ASSISTANT REFEREES AS WELL AS THEIR GRADE LEVELS before each match. You don't want to have to call up referees and assignors after matches for this information.  Try to be pro-active in this way.

No Passes - Sign-in Procedure - Youth Competitions

If no passes and/or roster are available prior to the start of the match; the Referee will have all players, coaches, and managers neatly print their names, birth dates, and sign their signature next to their entry. The head coach will verify that all information is correct and sign a statement that all players, managers, and coaches are of the proper age, are on the official roster, have CJSA passes, and are eligible to participate in the match. The opposing coach will be notified of the absence of roster and passes.
If the passes and/or roster arrive at the field before the end of the match, the Referee MAY elect to inspect and verify the passes at that time (either at half time or at the end of the match). If Referee determines that the roster and passes are in order, no further action is required.
If the passes are not available prior to the end of the game or the Referee elects not to verify the passes after the start of the match, the Referee will submit a game report with the handwritten roster to the District Office and CJSA Office within 48 hours.
